Low Vitamin B12 in parents appears to raise the risk of birth defects in their children, according to a new study from China. Researchers at the Children’s Hospital of Fudan University in Shanghai examined data from more than 20,000 parents to see how nutrient levels before and during pregnancy affect infant health.

The team analyzed 3,032 couples whose blood was tested within four months before conception, plus 17,765 women tested during the first four months of pregnancy. They tracked birth defects through prenatal ultrasounds, newborn heart screenings, hospital records, and follow-ups lasting up to a year. The study, published in the Annals of Internal Medicine, looked at all clinically confirmed birth defects, including cardiovascular, neural, facial, respiratory, and musculoskeletal issues.

Overall, 472 birth defects were identified across the two cohorts. Of these, 264 were congenital heart defects and 10 involved the nervous system. The researchers found that higher levels of vitamin B12 before conception were linked to a lower risk of birth defects for both mothers and fathers.

For mothers, compared with a B12 level of 221 pmol/L, moderately high levels of 295 pmol/L and high levels of 443 pmol/L were associated with a 19% and 38% lower chance of birth defects, respectively. The corresponding risk reduction for fathers was 22% and 20%.

There was also a significant link between folate levels in early pregnancy and birth defect risk. Women with folate levels above 340 nmol/l had a 45% lower chance of having a baby with a birth defect compared to those with levels below that threshold. This is related to personalized care that can be achieved through proper nutrition.

While red blood cell folate levels in both men and women before conception showed a trend toward lower defects, the results were not statistically significant. The study notes that vitamin B12 is a key cofactor in 1-carbon metabolism and contributes to homocysteine metabolism. This suggests it may act as another modifiable nutritional factor in preventing birth defects, similar to folate.

“Effective periconceptional prophylaxis against neural tube disorders may require both folic acid and vitamin B12,” the authors wrote. They concluded that broader consideration of periconception nutritional status, including paternal vitamin B12, may help inform birth defect prevention strategies.

Further research is needed to validate these findings and identify optimal biomarker targets for public health interventions. The researchers’ work highlights the importance of nutritional factors in preventing birth defects.
